19: PreOxi Trial (VNI para preoxigenação na emergência)
<p><strong>Curso</strong>: https://simulamedbr.com</p><br><p><strong>Caso: Você recebe na sua sala vermelha um paciente trazido da enfermaria no 5 dia de internação. Trata-se de paciente de 58 anos, masculino, previamente hipertenso, internado devido um quadro de AVCi fora da janela terapêutica para trombólise. Evoluiu hoje com quadro de taquidispneia(FR 30), dessaturação(Sat 96% em MNRl 15L), piora do rebaixamento do nível de consciência (ECGl 9) e febre. Você interroga um quadro de sepse por pneumonia broncoaspirativa. E além do protocolo de sepse, você também indica intubação para proteção de via aérea e suporte ventilatório.</strong></p><p><strong>No preparo para a intubação, você solicita que seja iniciada a preoxigenação com VNI. Porém recebe esses questionamentos: mas o paciente está rebaixado, e vai fazer a VNI só para intubar? Não é um risco, sem benefício?</strong></p><p><br></p><p><strong>E agora?
