Shamiram Feinglass: What Medicine Already Taught You About Leadership

Medicine teaches clinicians to triage crises, lead teams, make decisions with incomplete information, and learn openly from failure without ego. So why do we still act as though they arrive in the C-suite knowing nothing about leadership? In this episode of Leadership Rounds, Dr. Reena Pande sits down with Shamiram Feinglass, MD, MPH, a physician executive, public-health leader, Managing Director at Manatt Health, Sandoz board director, and co-author of The 60-Minute Clinician MBA. Their candid conversation covers what it actually takes to move from clinical expertise to enterprise healthcare leadership. Shami’s career has crossed nearly every part of the healthcare ecosystem: clinical medicine, public health, federal policy at CMS, the U.S. Public Health Service, global medical device and diagnostics leadership at Zimmer and Danaher/Beckman Coulter, pandemic response, governance, mental-health advocacy, and healthcare consulting. Today, she advises healthcare and life sciences organizations at Manatt Health while serving on the Sandoz board and as affiliated faculty at the Johns Hopkins Bloomberg School of Public Health. Her career also exposed a persistent problem: clinicians moving into executive roles are often underestimated because the business world fails to recognize how much leadership training is embedded in medicine. Clinicians themselves are rarely given a practical roadmap for filling the gaps. That frustration inspired Shami to co-author The 60-Minute Clinician MBA, a practical guide to financial literacy, strategy, operations, leadership, innovation, and career advancement for clinicians entering the C-suite. This conversation makes a compelling case: clinicians do not need to leave their clinical identity behind to become outstanding business leaders. They need to understand the leadership skills medicine has already given them, intentionally learn the ones it hasn’t, and become fluent enough in the language of business to bring their expertise fully to the table. The themes and personal examples come directly from the episode transcript. Why clinical training produces powerful (but often overlooked) executive skills How triage translates into business strategy Why clinicians need business fluency (but don’t need to become experts in every function) The difference between breadth of understanding and depth of expertise Why trust and relationships (not transactions) drive influence How leaders should make failures visible and “fail fast” Why Shami asked for an executive coach the moment she entered the industry What leading a global workforce through COVID taught her about crisis management and communications Why financial literacy is non-negotiable for clinician executives How clinicians can work effectively with finance, legal, operations, communications, and supply-chain leaders Why healthcare needs a stronger pipeline of physicians and clinicians prepared for enterprise leadership The importance of mentorship and “lifting as we rise”

Sharif Vakili: “Doctors Getting Replaced”? UpDoc’s CEO on a New Model for Clinical AI

The FDA just cleared an AI model to do something only licensed clinicians could do. The physician who built it explains what happens next. Dr. Reena Pande sits down with Sharif Vakili, MD, MBA, Stanford primary care physician and CEO/co-founder of UpDoc, weeks after his company earned the first FDA clearance for a software medical device with a patient-facing large language model: the first true clinical AI. Sharif traces his path from Yale to Johns Hopkins to Harvard Business School, through seven years in venture capital at Polaris Partners, to the idea he couldn’t let go of: closing the loop between remote patient data and actual treatment. Sharif Vakili is CEO and co-founder of UpDoc, a practicing primary care physician at Stanford Medicine, a former venture capitalist at Polaris Partners, and former host of The Doctor Is Out. UpDoc’s platform (validated in a Stanford-led trial published in JAMA Network Open) is deploying at Cleveland Clinic, Allegheny Health Network, and UCSF Health.Follow the show so you don’t miss next month’s conversation. Arpan & Amit Parikh: Brothers Building AI So You Never Forget What Your Doctor Said

What if remembering your doctor's advice wasn't your responsibility alone? Even with 13 physicians in their immediate family, brothers Amit and Arpan Parikh found navigating healthcare harder than it should have been. In this episode of Leadership Rounds, the physician brothers and co-founders behind Kin Health discuss building consumer-first healthcare experiences, founder-market fit, and why AI should work invisibly in the background to strengthen the patient-clinician relationship rather than replace it. Amit and Arpan trace their paths from clinical psychiatry to chief medical officer roles at high-growth behavioral health companies. Now, as founders of Kin Health, they dedicate their time and expertise to building and running a free patient app, developed by physicians to record medical visits, generate plain-language summaries, and help patients share and follow through on care instructions. Topics & Takeaways Why patients and caregivers need better tools after medical appointments The overlooked gap between clinical recommendations and patient recall What “founder-market fit” means in healthcare Why clinician executives are essential to AI product development How AI can power simpler, more human patient experiences The leap from CMO to founder Why patient-facing AI should work alongside the healthcare system, not against it Chapter Markers 00:00 — Two brothers, two psychiatrists, two physician founders 01:36 — Growing up around psychiatry 07:21 — The first leap from clinical practice to healthcare entrepreneurship 13:18 — Arpan’s path through addiction psychiatry, Wharton, and value-based care 18:20 — What Kin Health is building for patients and caregivers 19:53 — The family experience that sparked the idea for Kin 22:18 — Why this AI moment made Kin possible 24:17 — Why clinician executives matter in AI product development 25:23 — Founder-market fit in healthcare 30:19 — What physician product leaders bring to AI innovation 36:38 — Bringing AI to the patients who need it most 40:49 — Building alongside the healthcare system 42:05 — What gives Amit and Arpan optimism
