What Are the Benefits of Medical Massage for Pain Management?

In this episode of the New York Pain Medicine Podcast, we dive deep into the critical differences between a standard relaxation spa massage and clinical medical massage. If you have ever been told to simply "go get a massage" for chronic pain, you might be treating a structural musculoskeletal issue with a purely cosmetic fix. Join us as we explore how medical massage acts as a highly targeted, results-oriented therapy specifically designed to treat diagnosed clinical conditions and accelerate your physical rehabilitation. Rather than just unwinding for an hour, this therapy acts as a biomechanical reset for your body. In this episode, we cover: The Foundation Analogy: Why standard relaxation massage is like putting a fresh coat of paint on a house, while medical massage gets under the floorboards to fix a cracked foundation. Advanced Clinical Modalities: How therapists integrate targeted techniques like myofascial release, neuromuscular therapy, and trigger point therapy into a comprehensive pain management plan. Un-sticking the Fascia: The way myofascial release applies sustained pressure to bound connective tissue—similar to un-sticking Saran wrap—to restore free movement. The Science of Healing: How targeted intervention increases localized blood circulation, flushes out inflammatory chemicals, and breaks down rigid, restrictive scar tissue. Drug-Free Relief: The importance of manually correcting structural imbalances for non-invasive, drug-free relief from sports injuries, chronic pain, and post-surgical stiffness. Stop just surviving your symptoms and get to the root cause of your physical dysfunction. Ready to stop masking the pain? Book an appointment with Dr. Douglas Allen at New York Pain Medicine. How Does IV Ketamine Treat Both Neuropathic Pain and Fibromyalgia?

In this episode of the New York Pain Medicine Podcast, we explore how IV ketamine acts as a "hard reset" for the nervous system in patients suffering from treatment-resistant chronic pain. We break down the science behind the NMDA receptor to explain how ketamine blocks the malfunctioning distress signals that traditional oral medications and nerve blocks often miss. By comparing its effects on localized neuropathic pain—smoothing out hyperactive neural "ruts"—with its impact on systemic fibromyalgia—turning down the body's overactive pain "volume dial"—we reveal how this versatile treatment can effectively reprogram the brain's embedded memory of physical injury. What is Regenerative Medicine?

This episode of the New York Pain Medicine Podcast explores why some injuries, like a simple paper cut, heal in days while chronic joint pain can persist for a decade. We dive into the world of orthobiologics and how regenerative medicine is shifting the focus from masking symptoms to actually repairing damaged tissue. The Mechanic vs. The Warning Light Standard treatments for chronic pain often act like turning off a "check engine light" without fixing the motor. Regenerative therapies, however, function like hiring a mechanic to rebuild the engine by stimulating the body’s dormant healing mechanisms to repair tendons, ligaments, and joints. Key Therapies Discussed: Platelet-Rich Plasma (PRP): A concentrated sample of your own blood that uses growth factors to act as "biological flares," signaling stem cells to produce new collagen. Exosome Therapy: These "cellular foremen" deliver molecular blueprints directly to cells, providing specific instructions on how to synthesize proteins and calm inflammation. The Steroid Trap: While corticosteroids offer fast relief, they can shut down the vital first step of the healing cycle and potentially degrade joint integrity over time. What We Cover: How PRP density overcomes chronic inflammation. The difference between providing raw materials (PRP) and providing the blueprint (Exosomes). Conditions treated, including osteoarthritis, degenerative disc disease, and sports injuries. The surprisingly short downtime—most patients return to light activity immediately. What Is the Best Way to Recover from a Sports Injury?

When you tweak a muscle or feel that familiar twinge in a joint, your first instinct is usually to grab an ice pack and clear your calendar. But what if "resting and waiting" is actually the worst thing you can do for your long-term performance? In this episode, Dr. Douglas Allen breaks down the Physio Tree approach—a shift in mindset that moves recovery from a passive waiting game to an active engineering problem. We explore why generic physical therapy often fails athletes and how regenerative medicine, like PRP, can "speed up biology" to get you back on the field stronger than before. Key Takeaways The Mechanic vs. The Engineer: A GP might treat your symptoms (the flat tire), but a physiatrist investigates the biomechanical weakness (the alignment) to ensure you don’t "blow another tire" in three months. The Danger of "Babying" Injuries: Avoiding movement can lead to weak, inflexible scar tissue. Early, aggressive intervention ensures the body heals with high-quality, functional tissue. The Two-Phase Recovery Strategy: Phase 1: Put out the fire. Manage acute inflammation and pain to allow for initial movement. Phase 2: Speed up biology. Use regenerative treatments like Platelet-Rich Plasma (PRP) to provide a "supply drop" of healing factors to the injury site. Sport-Specific Physics: Recovery must match the sport. A runner’s rehab should focus on vertical impact and gait, while a golfer’s rehab must address rotational torque and grip. The Multimodal Toolkit: True healing requires layering treatments—combining nerve blocks, orthotics, athletic taping, and biological boosters to attack the problem from every angle.

Tired of Pills? A Guide to Acupuncture for Back Pain & Migraines

Are you tired of managing chronic pain with pills and worrying about side effects or dependency? In this episode, we explore the science behind medical acupuncture—a proven, non-surgical solution for conditions like sciatica, migraines, arthritis, and back pain. We dive deep into how acupuncture does more than just "block" pain. You'll learn the biological mechanisms at play, including how targeted needle placement triggers the release of endorphins (the body's natural painkillers) and calms an overactive nervous system to reduce inflammation. We also discuss the critical difference between spa acupuncture and physician-guided medical acupuncture, emphasizing the importance of an accurate diagnosis before treatment begins. Whether you are dealing with stubborn joint pain or looking for a safe alternative to opioids, this episode breaks down how integrating acupuncture into a comprehensive medical plan can help you regain mobility and improve your quality of life. In this episode, you will learn: The Biology of Relief: How acupuncture stimulates endorphins and regulates the nervous system. Targeted Treatment: Why local targeting of pain points is safer than systemic medication. Conditions Treated: Effectiveness for back pain, neck pain, sciatica, migraines, and myofascial pain syndrome. Safety & Integration: The benefits of a physician-guided approach that coordinates acupuncture with physical therapy and imaging. #MedicalAcupuncture #ChronicPainRelief #SciaticaTreatment #MigraineRelief #BackPain #NonOpioid #PainManagement #HolisticHealth #IntegrativeMedicine #NewYorkPainMedicine #PhysicianGuided Ready to find relief without surgery or medication? The Benefits of Ketamine for Chronic Pain Relief

Are you living with chronic pain that hasn't responded to standard treatments? Living with persistent pain can feel overwhelming, but you don’t have to face it alone. In this episode of the New York Pain Medicine Podcast, we explore Intravenous (IV) Ketamine Therapy, an innovative and powerful option emerging for patients who have exhausted traditional therapies. We explain what IV ketamine is and why it’s different: Unlike opioids, ketamine works on the NMDA receptor to help “reset” pain pathways and reduce the intensity of pain signals. Tune in to discover the five key benefits of IV Ketamine, including: • Its effectiveness for hard-to-treat pain conditions like Complex Regional Pain Syndrome (CRPS), Fibromyalgia, and neuropathic pain. • The rapid onset of relief that many patients experience within hours to days. • The advantage of being a non-opioid option that does not carry the same risk of dependency or tolerance. • The potential to improve mood, sleep, and overall quality of life due to its documented antidepressant properties. We also address frequently asked questions (FAQs) about safety (it is administered in low doses and continuously monitored by physicians), potential short-term side effects, and how to determine if this physician-guided, personalized treatment is the right step toward lasting relief for you. What Causes Back or Neck Pain?

Are you tired of nagging back or neck pain that affects your workday, your sleep, and the activities you love? Do you feel like you’ve been treating surface-level symptoms without getting to the root of the problem? In this episode, we sit down with Dr. Douglas Allen, a dual board-certified specialist in physical medicine, rehabilitation, and interventional pain management, to uncover his meticulous approach to solving chronic pain. Dr. Allen explains why the spot where you feel pain is often not the true cause and discusses the complex conditions that can lead to it, including pinched nerves, arthritis, sports injuries, and repetitive strain. Listeners will learn about Dr. Allen’s comprehensive diagnostic process, which is designed to provide clear answers and a precise diagnosis. The journey to relief begins with a crucial first step: listening to the patient's history and lifestyle, followed by a detailed physical exam to identify revealing patterns. We also dive into the advanced diagnostics he employs when necessary, such as: • Imaging like X-rays, CT scans, and MRIs to see inside the spine. • Electrodiagnostic studies (EMG/NCS) to assess nerve function. • Diagnostic tools like nerve blocks or discography to confirm if a specific nerve or disc is the pain generator. Once the exact source of pain is pinpointed, Dr. Allen creates a Personalized Pain Management Plan. This isn't a one-size-fits-all solution. Discover the range of targeted treatments available, from conservative options like physical therapy and acupuncture to advanced interventional procedures such as epidural injections and radiofrequency ablation. The goal of this holistic plan is not just to relieve pain but to improve mobility, strength, and your long-term quality of life.
