Fri 2026/10/09 Friday Consolidation: Anti-Amyloid Therapy, Button Batteries, Lipid Therapy Sequencing, Menopause Therapy

Hey! I'd love to hear your thoughts, send me a voice note. (https://rss.com/podcasts/santa-reads-to-you-about-medicine/voicemail) The weekly consolidation for the week of October fifth, covering four episodes — Monday's sweep on VESALIUS-CV, triglycerides, and two pediatric In Brief pieces; part two of the dementia series on management; pediatric foreign bodies; and part one of the menopause series on systemic hormone therapy. It leads with six things worth keeping: the anti-amyloid benefit in points rather than percent, the two-hour clock and honey for an esophageal button battery, the end of the requirement to use ezetimibe before a PCSK9 antibody along with the two settled triglyceride thresholds, the menopause label change and what it does and doesn't rest on, transdermal estradiol as the default, and pain, constipation and retention before any antipsychotic in dementia. It also reports corrections made this week to the lipid episode and to Monday's sweep before it aired. A second tier covers a dozen shorter keepers, from ARIA and ApoE4 to croup that's really an airway foreign body, the parent's kiss, and fezolinetant liver monitoring. It closes on four threads: relative numbers that sound larger than absolute ones, labels and marketing that move ahead of the trials, the common diagnosis that stops fitting, and which way the bias runs in observational safety data.

Thurs 2026/10/08 Approach: Menopause, Part One: HRT / Systemic Therapy

Hey! I'd love to hear your thoughts, send me a voice note. (https://rss.com/podcasts/santa-reads-to-you-about-medicine/voicemail) Part one of a two-part series on menopause, covering systemic hormone therapy for vasomotor symptoms, drawing on documents plus three April 2026 ACP Internal Medicine Meeting sessions (Monica Christmas, Lisa Larkin, and a UCLA speaker). It opens with the regulatory story — the November twenty twenty-five FDA announcement removing the boxed warnings for cardiovascular disease, breast cancer, and dementia, the first revised labels approved in February twenty twenty-six, the fact that implementation is still product by product, and the criticism of both the process and the benefit claims. It then goes back to what the Women's Health Initiative actually showed in absolute numbers and by age, and why the regimen it tested differs from what most clinicians prescribe now. It covers the new American College of Physicians guideline published October sixth, with its recommendation grades, its clinical considerations, and the accompanying Mayo Clinic editorial, including where the guideline departs from the Menopause Society and from the ACP meeting speakers on route of administration, family history of breast cancer, age cutoffs, and duration. Then the practical prescribing: candidacy and contraindications, transdermal versus oral and the venous thromboembolism data, doses, progestogen choice and endometrial protection, bleeding, duration and the end of the arbitrary stop date, perimenopause, and premature ovarian insufficiency. It closes with the nonhormonal ladder, including fezolinetant's liver monitoring and the newly approved elinzanetant, and a note on compounded hormones.

   Wed 2026/10/07 Pediatrics Foreign Bodies

Hey! I'd love to hear your thoughts, send me a voice note. (https://rss.com/podcasts/santa-reads-to-you-about-medicine/voicemail) A pediatric episode on foreign bodies — what children swallow, inhale, and put in their noses and ears, and what primary care needs to do about each. It opens with an October Pediatrics in Review case of a twelve-month-old treated twice for croup whose stridor turned out to be a dried leaf across the vocal cords. It covers airway foreign bodies, including why a normal chest film doesn't exclude one and why bilateral decubitus films don't help; ingestions, drawing on the April American Family Physician review and the Cribsiders episode with Gabriel Devlin, with how to tell a coin from a button battery on X-ray, the two-hour clock for an esophageal battery and the honey protocol en route, why multiple magnets and water beads are emergencies, and when watchful waiting is reasonable; nasal foreign bodies and the parent's kiss technique; ear foreign bodies and live insects; and prevention, including secured battery compartments.
