Approach to Spongiotic Derm: The First Four Visits

Dr. Matthew Zirwas presents a practical, step-by-step 4-visit algorithm for diagnosing and managing patients presenting with itchy rashes and non-specific spongiotic dermatitis. He outlines modern treatment strategies—moving from basic skin care regimens, barrier preservation (combining clobetasol solution with CeraVe cream), and low-irritancy laundry habits to advanced biologic therapies, JAK inhibitors, and patch testing. In addition, Dr. Zirwas demonstrates using UV light to detect scabies, discusses class-switching in biologic failures, and explores treatments for neuropathic itch using medications like Mirtazapine and Duloxetine.
