【病歷資訊管理教育訓練】醫療保險費用支付制度 EP197-18【醫療保險費用支付制度】
單元1：健康照護與醫療保險體系
醫療保險三大體系分類
1.社會醫療保險(SHI)
•代表國家：德國，1883《工人疾病保險法》，世界第一個社會保險；政府、雇主、民眾共同出資成立保險基金。
•我國全民健保即屬此類，強制納保；優點財務穩定、社會公平；缺點醫療開支膨脹會有財務風險，常搭配總額制度控費。
2.公醫制
•代表：英國NHS，1948實施；政府編列稅收預算，醫師類似公務人員。
•特色：看病免費；缺點官僚化、預算壓力大、就醫等候時間長，民眾轉向私立醫院。
3.自由市場制
•代表：美國；政府少介入，以民間商業保險為主。
•公保：Medicare紅卡(65歲以上、身心障礙)、Medicaid白卡(低收入)；民間HMO、PPO、EPO等。
•缺點：保費昂貴、部分民眾無保險，公平性不足。
單元2：保險機構支付費用的方法
依支付時間點區分
1.回溯性支付RPS：事後結算，代表：論量計酬FFS，做完服務之後再申報費用。
2.前瞻性支付PPS：事先訂定包裹式價格，不管實際花多少，依預定金額給付；代表DRG診斷關聯群。
常見六種支付方法
1.論量計酬 FFS：做多少項目給多少錢；缺點：容易誘發過度醫療、多做檢查與延長住院。
2.論日計酬 Per Diem：按住院每一天給固定金額，多見商業保險。
3.論病例計酬 Case Payment：同一個手術/病例給定單一點數；選用療程單純外科手術；制訂基本診療程式，避免醫院省略必要照護。
4.DRG診斷關聯群（美國） / Tw‑DRGs（臺灣版）
•美國1983 Medicare開始實施；依ICD‑10‑CM/PCS，考量疾病、處置、年齡、合併症、併發症、出院狀況，資源耗用相近歸同一組，給予相同給付。
•效果：平均住院日下降；醫院承擔財務風險；後續復健、護理之家使用上升。
5.論人計酬 Capitation：依照投保人口，每個人每年固定金額給醫院。醫院要做好預防保健，減少就醫才能獲利；與論量計酬剛好相反。我國有試辦計畫，分區域整合、社區醫療群、醫院忠誠病人模式。
6.總額預算制度 Global Budget：事先協商年度總預算。
•支付上限制（我國健保採用）：總額固定，服務量膨脹，每點價值就會被稀釋下降。
•支出目標制：設定成長上限，超過部分打折給付。
補充：論質計酬 Pay‑for‑Performance
以品質指標做給付誘因，針對糖尿病、氣喘、肺結核、癌症；指標分結構、過程、結果三大構面。
單元3：我國全民健康保險制度
1.沿革：1995‑03‑01正式實施全民健保；2013二代健保上路，導入補充保費；健保署建置雲端藥歷、健康存摺。
2.制度七大特點
1.強制全民納保，分6大類被保險人。
2.給付範圍廣：門診、急診、住院、中醫、牙科、復健、分娩。
3.就醫自由：可自由選擇特約醫事機構。
4.部分負擔機制：鼓勵分級醫療；重大傷病5年免部分負擔；住院有部分負擔上限。
5.多元支付制度：論量、論病例、Tw‑DRGs、總額、論人、論質計酬。
6.二代健保：家戶所得計算保費、補充保費（獎金、股利、租金、執行業務所得等）。
7.健康存摺：民眾可查詢就醫、檢驗、用藥、預防保健紀錄。
單元4：我國全民健保各項支付制度
1.論量計酬：早期主要制度；缺點容易造成服務量膨脹。
2.論病例計酬：以手術病例為主；設上下除外臨界點，超出上限部分僅給付60%；未執行基本診療程式會扣減費用；2週內同病因再住院要重新專業審查。
3.Tw (http://3.Tw)‑DRGs臺灣診斷關聯群：參考美國DRG；疾病、手術、合併症、併發症會改變DRG組別與相對權重RW；病歷書寫完整、正確編碼才能歸入正確DRG拿到合理給付。
4.總額預算制度：1998逐步推，2002全面實施；搭配品質確保方案，監測就醫權益、專業品質、健康指標、醫療利用。
5.論人計酬試辦：三種模式，重預防保健、慢性病管理。
6.論質計酬：品質導向，財務誘因鼓勵良好照護。
單元5：保險費用申報
申報流程
1.院所每月20日前申報上個月醫療費用；上傳費用、醫令、病歷佐證資料。
2.健保署兩種審查
•程式審查：電腦檢查格式、規則。
•專業審查：抽樣病歷，由醫療專家審查醫療適當性。
3.病歷是申報最重要佐證，病歷紀錄缺失會造成健保核減。
常見病歷缺失導致核減
•紀錄簡陋、資料不全、前後矛盾、塗改、侵入性處置沒有理由、缺少同意書、出院摘要延遲、複簽不足、次要診斷/合併症/併發症漏記、編碼與病歷不符、大量複製貼上。
病歷資訊管理人員、疾病分類人員職責
1.確保病歷內容可以佐證申報項目。
2.依病歷做正確ICD編碼，禁止Up‑coding（向上編碼，為拿更高給付選高代碼），遵守編碼倫理。
3.熟悉申報規則，協助醫師寫好病歷，減少核減損失。
單元6：健保資料庫加值運用
1.NHIRD全民健康保險研究資料庫：由國衛院建置；原始申報資料經過二代加密（去識別化兩次加密），保護個人隱私。
2.主要檔案：基本資料檔、醫令檔、費用檔；另有各種特定主題分檔（癌症、糖尿病、重大傷病、中醫、牙醫等）。
3.加值運用：學術研究、政策分析；樣本數龐大、涵蓋幾乎全國國民，代表性高；屬於目的外利用。
